CodifyMed services and associated functionalities are not meant for use by minors without supervision. By accessing, using, and/or submitting information into the service, you confirm that you are 18 years of age or older, or that you are using the service with the involvement of a responsible adult.
For users under the age of 18, we always require guarantor information and a guarantor’s signature. This ensures that a responsible adult, such as a parent or legal guardian, is responsible for the minor’s healthcare information.
What We Collect and What We Do with It
If you are a patient using a service provided by a practice that works with CodifyMed, your information will be securely transmitted to the practice you selected.
The combination of your name, email, phone number/zip code, and date of birth (DOB) will be used to uniquely identify you to the practice, allowing them to determine whether you are a new or existing patient.
Your “reason for visit” will be transmitted to the practice so they can plan accordingly and confirm that the visit is appropriate.
We may use your phone number and email address to send appointment confirmations, reminders, and other messages related to your visit. Upon registration, you have the option to select “I consent to receive text messages from the practice,” allowing us to send you important notifications related to your healthcare experience.
If payment for a visit is required, we may securely transmit your information to financial partners to facilitate the payment on behalf of the practice.
We do not:
- Sell patient or practice data to third parties
- Run ads on our platform
- Use cookies to track patients across other sites
Data Retention
CodifyMed will maintain practice data for as long as the practice remains a customer of CodifyMed.
If a practice discontinues using CodifyMed services, all of the practice’s data, including the practice’s name, contact information, and any other associated details, will be automatically deleted from our systems within 7 days.
How We Protect Your Information
All data traffic for services provided by CodifyMed is encrypted via SSL, ensuring that your personal information is secure during transmission.
When we send emails regarding appointments, we will include your name, email, practice name and the name of the provider, but we will not include your date of birth or any medical information to protect your privacy.
Third Party Disclosure
We do not sell, trade, or transfer any personally identifiable information to marketing or advertising entities.
This does not include the following, who may receive your information as part of our services:
- Any medical practices we are securely transmitting information to on your behalf.
- Website hosting partners and other parties who assist us in operating our website, conducting business, or providing services to you, as long as these parties agree to keep your information confidential.
We are committed to ensuring that all data handling is in line with privacy regulations and that no personally identifiable information is used for marketing, advertising, or other unrelated purposes.
